Eligibility
You are eligible for home health care if:
You are 65 years or older
Have been diagnosed with a new illness
Your medication has recently changed
You require nursing and therapy services and would rather receive this at home
Have been hospitalized recently
Feel you are at risk for a fall
Homebound status
Under the care of a Primary Care Physician
Contact us to evaluate your eligibility and we will coordinate home health authorization with your Primary Care Physician
